Volunteer Clearances

VOLUNTEERS WHO HAVE LIVED IN PA FOR MORE THAN 10 YEARS

Library Volunteers/Workers that have lived in PA for more than 10 years will need to fill out the following forms below...

PA Child Abuse Form

PA State Police Criminal History Form

Disclosure Statement --- Must be printed out & completed with a witness signature at the Library

**Volunteers need to SUBMIT the results from these clearances to the Library Desk
